Job Description
The Diabetic Educator will provide expert guidance and education to patients with diabetes in both in-person and virtual outpatient settings. The role involves developing personalized care plans, educating patients on self-management techniques, and collaborating with healthcare professionals to optimize patient outcomes. The ideal candidate is passionate about patient education, skilled in both individual and group settings, and proficient with virtual healthcare technologies.
**Key Responsibilities:**
- Conduct in-person and virtual consultations with diabetic patients to assess their educational needs.
- Develop individualized care plans based on patient assessments, medical history, and treatment goals.
- Provide education on diabetes management, including medication administration, blood glucose monitoring, nutrition, exercise, and lifestyle modifications.
- Utilize virtual healthcare platforms to deliver education and support to patients remotely.
- Organize and lead group education sessions, workshops, and support groups for diabetic patients and their families.
- Collaborate with physicians, nurses, dietitians, and other healthcare professionals to coordinate comprehensive care for diabetic patients.
- Monitor patient progress, adjust care plans as needed, and provide ongoing support to ensure successful diabetes management.
- Maintain accurate and up-to-date patient records, documentation, and reports.
- Stay current with the latest developments in diabetes care and education to provide the highest quality of service.
- Participate in community outreach programs to raise awareness about diabetes prevention and management.
**Qualifications:**
- Bachelor’s degree in Nursing, Dietetics, Public Health, or a related field; Master’s degree preferred.
- Certified Diabetes Educator (CDE) or Diabetes Care and Education Specialist (DCES) certification required.
- Minimum of 2 years of experience in diabetes education or related clinical practice.
- Proficient in using telehealth and virtual care platforms.
- Excellent communication and interpersonal skills.
- Ability to develop strong relationships with patients and collaborate effectively with a multidisciplinary healthcare team.
- Strong organizational skills and attention to detail.
- Ability to work independently and manage a flexible schedule that includes both in-person and virtual care.
