Early Intervention Transition Coordinator - Full Time Schedule Monday- Friday Pay $52,000.00 per year (This is an hourly position/ Based on a 40 hour week) Are you looking to make a difference in a child's life? The Early Intervention program of People, Incorporated is looking for a full-time Early Intervention Transition Coordinator. The employee provides oversight of all children exiting Early Intervention services, including those transitioning to local school systems.
ESSENTIAL FUNCTIONS :
•Oversee all aspects of transition planning activities for children exiting EI services, ensuring compliance with DPH policies and timelines.
•Work closely with the Intake Coordinator to ensure that all late referrals are completed timely and appropriately.
•Support service coordinators and families through the referral process by assisting with transition planning and/or referrals as appropriate and as determined by the families' interests and goals.
•Participate in transition planning conferences as required.
•Ensure that parents are provided a copy of Family Rights and notified of their ability to opt out of notification to SEA or LEA if their child is potentially eligible for Part B services.
•If parental consent for an LEA referral is given, provide documentation and pertinent records in a timely manner to avoid duplication of evaluations, support the process for determining Part B eligibility, contribute to the development of an Individual Education Plan (IEP), and facilitate continuity of services for children turning 3.
•Upon invitation of the parent and as feasible, attend the child's IEP meeting.
•Maintain positive working relationships with early education and special needs team leaders at school districts within the Fall River catchment area. Attend transition network meetings to collaborate with school districts and EI programs in the Southeast.
•Provide regular updates and reports to Program Director and Supervisors on all transition-related activities. •Provide training to all service coordinators on the required process and timelines.
•Provide direct Early Intervention services for assigned cases.
•Maintain assigned productivity level on a monthly basis.
•Perform other duties as assigned by the supervisor, division management team member, or division director, including, but not limited to, working weekends and/or holidays.
•Participate in all staff meetings and required training sessions, including in-service training, as scheduled.
•Maintain all program and Agency required trainings.
QUALIFICATION REQUIREMENTS :
- Bachelor's degree in related field required.
- Knowledge and skill obtained through a minimum of three years of Early Intervention experience
- Skill in communicating and working with others
- Ability to maintain productivity
- Ability to participate in program planning and implementation cooperatively with the Program Director, Assistant Director, and Program Supervisors in a positive and solution-oriented manner
- A qualifying criminal background check, approved driving record, and a valid driver's license required
- Ability to provide current copy of all required health records
