The Health and Wellness Director is the heart of the clinical department. Your work will ensure our residents receive the highest standard of care. Instilling a positive and inclusive community culture, setting and attaining nursing department goals, inspiring your clinical team, creative problem solving and overseeing all aspects of the clinical department are all essential parts of your position here at Plum Creek. Our residents will depend on your knowledge, skills and attention to detail to ensure they receive an unparalleled standard of care.
E ssential Duties:
- Directly oversee and assist with the management of staff and residents with the primary focus of serving the healthcare needs of residents in accordance with federal and state regulations/standards.
- Exhibit professional and ethical work practices.
- Adhere to all policies and procedures.
- Work with staff to ensure that high standards of care and customer service are delivered as per resident preferences and plans of care.
- Complete the comprehensive evaluation on potential residents and current residents for admission criteria.
- Monitor documentation systems to ensure that all regulatory timeframes for review of evaluations and service plans with regards to resident needs are being met.
- Monitor documentation from home health, hospice and ancillary care providers, to ensure appropriate communication of services provided and to ensure the needs of the residents are being met.
- Advise management personnel to be aware of resident health issues for which they may need to observe, report or take specific measures.
- Provide oversight and management to the medication management program within the community.
- Responsible for monitoring key quality performance metrics within their department; collaboration with leaders on plan development to achieve improvement in quality of care and compliance outcomes.
- Responsible for ensuring that the department is appropriately staffed to meet the health and wellbeing of residents which may include management and/or oversight of the staffing schedule.
- Supervise wellness staff, including their schedule, on-time attendance, performance as a team member, individual performance of daily duties and responsibilities and attitude and compassion with the residents.
- Plan, schedule and conduct staff training programs and in-services as needed.
- Strive to maintain a safe working environment through the prevention of accidents, the preservation of equipment and the achievement of safe working practices.
- Ensure that all concerns and incident reports are completed, filed and that proper follow up is conducted timely.
- Must maintain and encourage a positive work atmosphere and communicate in a manner that promotes cooperation and quality experiences for co-workers, managers and others.
- Must demonstrate empathy and compassion to connect with staff, family members and residents. Must be inspired to serve the needs of our customers.
- Promote a positive customer service experience by maintaining a clean work environment.
- In conjunction with the Executive Director, address resident grievances in a timely manner.
- Must be willing to be on call 24/7
- Perform other duties as assigned.
Q ualifications:
- Must be licensed as a Registered Nurse with a license in good standing.
- Maintain an active CPR certification
- Maintain CEU's as per state license renewal requirements
- Minimum of three years nurse management experience including hiring, coaching, performance management and clinical operation supervision.
- Knowledge of applied nursing practices and techniques in accordance with federal, state and local regulations.
- Experience with assessments, survey management, medication management etc.
- Must be able to stand, walk, stoop and/or bend for periods of up to 8 hours with breaks as provided for in the employee handbook.
- Experience with EMR/EHR clinical documentation systems
- Exude energy and enthusiasm with a hospitality focused mindset.
- A passion for serving seniors and growing teams
- Maintains a positive and friendly demeanor toward residents and co-workers.
- Self-awareness, organization and time management skills
- Must have a valid drivers license with a clean driving record.
- Ability to multitask and have excellent attention to detail
- Possible exposure to communicable diseases, infectious diseases and blood borne pathogens.
- Familiarity with Supportive Living/Assisted Living population preferred
