Job Description

Spero Hospice in Middlesex, NJ is seeking a compassionate and dedicated Hospice Nurse - LPN to join our team.   • Provide compassionate and comprehensive nursing care to terminally ill patients in their homes or in hospice facilities.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans for each patient.
  • Administer medications and treatments as prescribed by the physician to manage pain and symptoms effectively.
  • Educate patients and their families on end-of-life care, symptom management, and comfort measures.
  • Maintain accurate and thorough documentation of patient assessments, care provided, and outcomes in compliance with regulatory requirements.
Qualifications:
  • 2+ years of experience in hospice care
  • Valid LPN license in the state of New Jersey
  • Strong communication skills for interacting with patients and their families
  • Ability to provide compassionate end-of-life care
  • Proficiency in administering medications and monitoring patient conditions
