Overview:

We are seeking a compassionate and dedicated Home Health Nurse to join our team. The ideal candidate will provide essential assistance to patients in their homes, ensuring their comfort and well-being. At ALLHEART HHC we look for nurses who are compassionate, adaptable, detail oriented and will serve as efficient advocates for our patients.

Duties:

• Assess and document patient condition at each visit.
• Complete evaluation of patient safety, functional status, medications, etc,.
• Schedule and attend patient appointments.
• Communicate with interprofessional team and patient.
• Monitor patient condition(s) and report any changes to PCP.

Requirements:

• Experience in related field is preferred.
• Strong interpersonal and interprofessional skills.
• Ability to act quickly, safely and efficiently in emergencies.
• Flexibility and ability to work in a variety of environments.
• Punctuality and efficient time management.
• Valid Drivers license, ability to pass federal background check and willingness to travel within surrounding area.
