Job Title: Psychiatric Home Health Registered Nurse
Job Summary:
We are seeking a compassionate and experienced Psychiatric Home Health Registered Nurse to join our team. The ideal candidate will be responsible for providing high-quality psychiatric nursing care to patients in their homes. The Psychiatric Home Health Registered Nurse will work closely with patients, families, and other healthcare professionals to develop and implement individualized care plans.
Responsibilities:
- Conduct comprehensive psychiatric assessments of patients in their homes
- Develop and implement individualized care plans based on patient needs and goals
- Administer medications and treatments as prescribed by the physician
- Monitor and document patient progress and report any changes to the physician
- Educate patients and families on mental health conditions, treatment options, and self-care techniques
- Collaborate with other healthcare professionals to ensure coordinated and effective care
- Maintain accurate and up-to-date patient records

Requirements:Requirements:
- Active and unrestricted Registered Nurse license in the state of practice
- Minimum of 2 years of experience in psychiatric nursing
- Experience in home health or hospice care preferred
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Compassionate and patient-centered approach to care
- Reliable transportation and willingness to travel to patient homes
